      目的:分析电针结合屈伸肌交替电刺激治疗与常规电针治疗对脑卒中患者下肢功能的影响,为临床脑卒中遗留下肢功能障碍的康复提供优化的治疗方案。方法:将符合本研究纳入标准的60例患者随机分为对照组与试验组,每组30例。本课题进行中两组受试者首先给予针灸科常规治疗,对照组患侧下肢接受电针干预,取穴:患侧阳陵泉、足三里、悬钟、丘墟,针刺得气后连接电针治疗仪导线,刺激频率2Hz,连续波,电流强度患者耐受为度,每日1次,每次30min,治疗6天,休息1天,持续28天;治疗组接受电针结合屈伸肌交替电刺激干预,电针治疗取穴同对照组,低频电刺激取穴:患侧阳陵泉、悬钟、承山、昆仑;刺激频率50Hz,断续波,屈伸肌交替刺激,屈伸刺激比为(1:2),每次治疗20min。每日1次,治疗6天,休息1天,持续28天。治疗前后采用RM Gait步态数据采集分析系统、Fugl-Meyer量表、Tinnetti平衡步态评分进行疗效判定。治疗前后数据统计采用SPSS 23.0进行分析。结果:治疗28天后,两组患者患侧步态参数(步长、步速、步频、患侧单支撑相)、下肢Fulg-Meyer评分、Tinnetti平衡步态评分比较疗前明显改善,组内比较差异具有统计学意义(P<0.01),组间比较差异具有统计学意义(P<0.05),治疗组疗效优于对照组。结论:电针结合屈伸肌交替刺激法治疗与电针治疗均可改善脑卒中后下肢功能障碍,电针结合屈伸肌交替刺激法疗效更佳。

      Objective:To compare the effect of EA combined with alternating stimulation of flexion and extensor muscle and conventional EA on lower limb function in Hemiplegia after Stroke, and to provide an optimized treatment scheme for the rehabilitation of lower limb function in clinical. Methods:Sixty hemiplegia patients after stroke with walking disorder were adopted and randomly divided into treatment group and control group, within 30 cases each group. During the project both two groups of subjects accept basic rehabilitation. The control group subjects were selected acupoints (the affected side zusanli,yanglinquan; xuanzhong,qiuxu), received EA therapy, frequency is 2Hz,continuous wave, 30 minutes each time and once a day, with one day rest every six day,lasting 28d. The treatment group subjects received EA combined with flexors and extensors alternation stimulation treatment.stimulating acupoints (the affected side:yanglingquan, xuanzhong; chengshan,Kunlun).frequency is 50Hz, discontinuous wave, flexor and extensor stimulation ratio is 1:2,20 minutes each time,and once a day,lasting 28d. RM Lab Gait analysis system, Fulg-Meyer score, Tinetti Performance Oriented Mobility Assessment were used to evaluate the treatment effects. SPSS 23.0 statistical software was used to analysis the datas. Results:End of the 28d treatment, the gait parameters of patients (step length, step speed, step frequency, single supportive phase on the affected side) and the lower limb fulg-meyer score and the Tinetti Performance Oriented Mobility Assessment of the two groups were significantly improved before the treatment,the differences between the treatment group and the control group were statistically significant (P < 0.05). The therapeutic effects of the treatment group were better than the control group. Conclusion:Effect of EA combined with flexor and extensor alternative stimulation treatment is better than the EA treatment on improving the function of walking ability.
