      目的:观察通元针法联合隔药盐灸神阙对产后抑郁症模型大鼠下丘脑-垂体-性腺轴相关激素水平的影响,探索针灸治疗产后抑郁症的疗效机制。方法:雌性SD大鼠120只,随机分为6组,每组20只;空白组不予处理,假手术组实行不摘除卵巢的假手术,模型组、药物组、常规针刺组、实验组参照文献建立产后抑郁症模型;造模成功后,模型组不干预治疗,药物组采用盐酸氟西汀治疗,常规针刺组电针百会穴、太冲穴治疗,实验组采用通元针法联合隔药盐灸神阙治疗;在干预治疗开始后的第2周末、第4周末每组各随机选取大鼠10只,检测大鼠的行为学指标后处死,检测大鼠血清GnRH、FSH、LH、E2、P水平。结果:(1)模型组大鼠的糖水消耗量、旷场实验水平和垂直活动评分均低于空白组(P<0.05),强迫游泳实验不动时间长于空白组(P<0.05);治疗后药物组、常规针刺组、实验组大鼠的糖水消耗量以及旷场实验水平和垂直活动评分高于模型组(P<0.05),强迫游泳实验不动时间短于模型组(P<0.05)。(2)干预后第2W、4W,空白组、假手术组血清GnRH、LH、FSH、E2、P均无显著差异(P>0.05);药物组、常规针刺组、实验组血清GnRH、E2显著高于模型组(P<0.05),血清LH、FSH、P显著低于模型组(P<0.05)。干预后第4W,实验组血清GnRH、E2与空白组无显著差异(P>0.05),显著高于药物组、常规针刺组(P<0.05);实验组血清FSH实验组、药物组与空白组无显著差异(P>0.05),显著低于常规针刺组(P<0.05);实验组血清LH、P药物组、常规针刺组、实验组间、空白组间无显著差异(P>0.05)。结论:通元针法联合隔药盐灸神阙能有效改善产后抑郁症模型大鼠的抑郁症状,作用机制可能与调节下丘脑-垂体-性腺轴相关激素水平有关。

      Objective:To observe the effect of Tongyuan-Acupuncture Combined with Medicine-Salt Moxibustion of Shenque on the levels of hormones related to hypothalamus-pituitary-gonad axis in postpartum depression model rats, and to explore the therapeutic mechanism of acupuncture and moxibustion for postpartum depression.Methods: 120 female SD rats were randomly divided into 6 groups, 20 in each group; the blank group was not treated, and the sham operation group was treated with sham operation without ovariectomy. The model group, drug group, conventional acupuncture group and experimental group established postpartum depression model with reference to literature. After successful modeling, the model group did not intervene in treatment, the drug group was treated with fluoxetine hydrochloride, and the conventional acupuncture group was treated with electro-acupuncture at Baihui and Taichong points. In the experimental group, 10 rats were randomly selected from each group at the 2nd and 4th weekends after the intervention. The rats were sacrificed after the behavioral indicators were tested, and the serum GnRH, FSH, LH, E2 and P levels were detected. Results: (1) Sugar and water consumption, open-field horizontal and vertical activity scores of rats in model group were lower than those in blank group (P<0.05), and the immobility time of forced swimming experiment was longer than that in blank group (P<0.05); after treatment, sugar and water consumption of rats in drug group, conventional acupuncture group, experimental group and open-field horizontal and vertical activity scores of rats in model group were higher than those in model group (P<0.05), while forced swimming experiment was not. The movement time was shorter than that of model group (P<0.05). (2) The serum GnRH, LH, FSH, E2 and P in blank group and sham operation group were not significantly different at 2W and 4W after intervention (P>0.05); the serum GnRH and E2 in drug group, routine acupuncture group and experimental group were significantly higher than those in model group (P<0.05), and the serum LH, FSH and P were significantly lower than those in model group (P<0.05). At 4W after intervention, there was no significant difference in serum GnRH and E2 between experimental group and blank group (P>0.05), which was significantly higher than that of drug group and conventional acupuncture group (P<0.05); there was no significant difference in serum FSH between experimental group, drug group and blank group (P>0.05), which was significantly lower than that of conventional acupuncture group (P<0.05); there was no difference in serum LH, P drug group, conventional acupuncture group, experimental group and blank group. Significant difference (P>0.05). Conclusion: Tongyuan-Acupuncture Combined with Medicine-Salt Moxibustion of Shenque can effectively improve the depressive symptoms of postpartum depression model rats. The mechanism may be related to the regulation of hypothalamus-pituitary-gonad axis related hormones.
