| 目的：利用表面肌电图(sEMG)观察针刺项丛刺主穴风池穴对吞咽障碍患者的即刻效应。方法：选择符合入组标准的60例脑卒中吞咽障碍患者,随机分为试验组30例、治疗组30例,均接受吞咽功能训练。试验组在此基础上接受项丛刺疗法治疗,1个月后对比两组患者标准吞咽功能评价量表(Standardized Swallowing Assessment SSA)评分情况以及电视透视吞咽功能检查(video fluoroscopic swallowing study,VFSS)评分,并首次利用sEMG探讨针刺项丛刺主穴风池穴对吞咽5ml水的即刻变化,初探针刺风池穴即刻治疗吞咽障碍的作用机制。结果 两组患者SSA评分均较治疗前减低,VFSS 评分较治疗前提高(P＜0.05),且试验组均优于对照组(P＜0.05)。sEMG示针刺风池穴即刻可提高颏下肌群、舌骨下肌群振幅均方根值(RMS)(均P＜0.05)。结论 项丛刺疗法结合吞咽功能训练治疗吞咽障碍更有效,通过针刺风池穴即刻可影响吞咽相关肌肉,提高肌肉振幅并减少吞咽时间。 |

| Objective The Application of Surface Electromyography in Functional Assessment of Immediate effect of puncturing the main acupoint Fengchi of nape-aligned acupuncture on Dysphagia Patients Methods A total of 60 patients of dysphagia were randomly assigned into a control group and an observation group,each 30 cases. The swallowing function treatment was provided to both 2 groups. Additionally,nape-aligned acupuncture was used in the observation group.The scores of the standardized swallowing assessment (SSA) and the video fluoroscopic swallowing study(VFSS) were observed after 1 mouth,and for the first time to explore immediate effect of swallowing 5ml water after puncturing the main acupoint Fengchi of nape-aligned acupuncture using surface electromyography(sEMG). Results SSA scores of both 2 groups were lower than those before treatment and VFSS scores of two groups were higher than those before treatment(P＜0.05). The above scores of observation group were better control group (P＜0.05). sEMG showed immediate effect of puncturing Fengchi could increase amplitude of the root mean square(RMS)of both submental muscles and infrahyoid muscles(P＜0.05). Conclusion Therapeutic effects of nape-aligned acupuncture combined with swallowing function training were more effective,and immediate effect of puncturing Fengchi is to shorten time and increase motion amplitude of dysphagia patients. |
