| 目的：应用Meta分析法系统评价雷火灸治疗膝骨关节炎的临床疗效及安全性；方法：计算机检索CNKI、WF、VIP及Pubmed数据库中雷火灸治疗KOA的临床随机对照试验文献，按照拟定的纳入标准和排除标准对原始文献进行筛选，运用Review Manager 5.3软件对纳入研究文献进行Meta分析；结果：最终纳入10篇文献，涉及987例患者，其中治疗组508例，对照组479例，2项为高质量研究，8项为低质量研究。结果显示：较之对照组，雷火灸在降低WOMAC评分无显著差异（P=0.14）；而在总体有效率、治愈率及改善VAS评分方面，雷火灸的治疗效果更好[OR=3.19’95％CI（2.07,4.90）’P＜0.000,01]、[OR=2.63’95％CI（1.68,4.11）’P＜0.0001]、[MD=-1.66’95%CI（-2.16,-1.16)’P＜0.000,01]；结论：雷火灸在KOA治疗上有一定的疗效，与药物治疗相比，其在总体有效率、治愈率、VAS评分、Lequesne评分及不良反应等方面，雷火灸更有优势，其在改善WOMAC评分方面仍有待观察。但由于纳入研究的样本量有限、文献质量普遍不高及可能存在偏倚等因素，今后尚需更多多中心、大样本的高质量及方法学规范化的随机对照研究加以验证。 |

| Objective: to evaluate the clinical efficacy and safety of thunder-fire moxibustion in the treatment of knee osteoarthritis by Meta-analysis. Methods: the clinical randomized controlled trial literature of KOA treated by thunder-fire moxibustion in CNKI,WF,VIP and Pubmed database was searched by computer, and the original literature was screened according to the proposed inclusion standard and exclusion standard. Review Manager 5.3 software was used for Meta-analysis of the literature included in the study. Results: a total of 987 patients were included in 10 articles, including 508 patients in the treatment group and 479 patients in the control group. 2 were high quality studies and 8 were low quality studies. There was no significant difference in WOMAC score between thunder-fire moxibustion group and control group (P>0.05). In terms of overall effective rate, cure rate and improvement of VAS score, the therapeutic effect of lightning moxibustion was better [OR=3.19'95%CI (2.07, 4.90)'P<0.000,01],[OR=2.63'95%CI (1.68,4.11)'P<0.0001]. [MD=-1.66'95%CI (-2.16, -1.16)'P<0.000, 01]; Conclusion: thunder-fire moxibustion has certain curative effect on KOA treatment. Compared with drug treatment, thunder-fire moxibustion has more advantages in overall effective rate, cure rate, VAS score, Lequesne score and adverse reactions, and it still needs to be observed in improving WOMAC score. However, due to the limited sample size, the general low quality of literature and the possible bias, there is still a need for more centers, high quality of large samples and standardized randomized controlled studies. |
