      目的:观察毫火针对膝骨关节炎模型大鼠软骨稳态的调节作用,并进一步分析其可能存在的分子机制。方法:SD大鼠随机分为空白对照组、模型组、毫火针组和普通毫针组,每组12只。用4%的木瓜蛋白酶溶液注射右膝关节造成炎性微环境模型。从造模后1周开始,毫火针组和毫针组取患侧的“足三里” “犊鼻” “内膝眼”进行点刺,每周3次,1个月后观察疗效。期间对各组大鼠爪压和步态进行评分;分子生物学方法检测软骨特异性基因和蛋白;ELISA检测关节液中白细胞介素-1β、肿瘤坏死因子-α。采用甲苯胺蓝染色观察软骨病例组织形态。结果:①经行为学观察,模型组步态和爪压评分较空白组均有升高(P<0.05,P<0.01);毫火针组与毫针组较模型组均显著下降(P<0.05);毫火针组评分低于毫针组,但差异不明显。②经分子生物学检测,与空白组比较,模型组软骨特异性指标COL-Ⅱ和SOX9显著下降(P<0.05,P<0.01),炎症相关指标IL-1β和TNF-α显著升高(P<0.05,P<0.01);与模型组比较,毫火针组的COL-Ⅱ和SOX9明显升高,升高水平毫火针组高于单纯的毫针组,毫火针组IL-1β和TNF-α均明显降低,其表达水平毫火针组低于毫针组(P<0.05,P<0.01)。③经组织切片染色可知,空白组关节面平滑,以透明软骨为主。模型组关节面粗糙,后期恢复大部分以纤维软骨为主,极易崩塌发展为骨关节炎并持续循环加重。经毫火针治疗的关节软骨平面明显优于模型组和单纯的毫针组。结论:毫火针可以减缓软骨组织的退变速度,对维持软骨稳态有一定的积极作用。

      Objective: To observe the regulating effect of mild fire Acupuncture on cartilage homeostasis in rats with knee osteoarthritis, and to further analyze its possible molecular mechanism. Methods: SD rats were randomly divided into blank control group, model group, filiform needle group and ordinary filiform needle group, 12 rats in each group. The right knee joint was injected with 4% papain solution to create an inflammatory microenvironment model. From 1 week after modeling, the filiform needle group and filiform needle group were punctured the affected side of "Zusanli Dubi Neixieyan ①After behavioral observation, the scores of gait and paw pressure in the model group were higher than those in the blank group (P<0.05, P<0.01); both the filiform needle group and the filiform needle group were significantly lower than those in the model group (P<0.05). ; The score of the filiform needle group was lower than that of the filiform needle group, but the difference was not significant. ② Compared with the blank group, the cartilage-specific indexes COL-Ⅱ and SOX9 in the model group were significantly decreased (P < 0.05, P < 0.01), and the inflammation-related indexes IL-1β and TNF-α were significantly increased (P < 0.05, P < 0.01); compared with the model group, COL-II and SOX9 in the filiform needle group were significantly increased, and the elevated levels in the filiform needle group were higher than those in the simple filiform needle group. significantly decreased, and its expression level in the filiform needle group was lower than that in the filiform needle group (P<0.05, P<0.01). ③ The tissue section staining showed that the articular surface of the blank group was smooth and mainly hyaline cartilage. The articular surface of the model group was rough, and most of the recovery in the later stage was mainly fibrocartilage, which was easy to collapse and develop into osteoarthritis and continued to circulate and aggravate. The articular cartilage plane treated with filiform fire needle was significantly better than the model group and the simple filiform needle group. Conclusion: Feilihuo needle can slow down the degeneration of cartilage tissue, and has a certain positive effect on maintaining cartilage homeostasis.
