      脊髓损伤(Spinal cord injury,SCI)包含复杂的病理生理学机制,SCI后免疫反应介导的神经炎症已成为研究热点,是继发性损伤中重要机制。血管破坏和血脑屏障(BBB)/血脊髓屏障(BSCB)破坏是免疫应答介导神经炎症的主要诱因,涉及免疫器官、免疫细胞和免疫因子的激活,导致损伤局部神经元、神经胶质细胞等代谢功能障碍。目前针刺治疗SCI在临床应用中已有显著疗效,作用机制与调节神经炎症相关的免疫细胞有关,包括调控中性粒细胞、小胶质细胞、淋巴细胞、星形胶质细胞等细胞活化表型和细胞因子的表达,从而有效降低SCI后炎症微环境并促进神经功能修复。本文系统地总结近年来国内外对SCI后神经-免疫炎症的作用机制的研究,并详细阐述针刺作为有效地神经免疫疗法的可行性依据,为针刺作为多效应靶点疗法在临床广泛应用提供理论支撑。

      Spinal cord injury (SCI) involves complex pathophysiological mechanisms. Neuroinflammation mediated by immune response after SCI has become a research hotspot, and it is an important mechanism in secondary injury. The destruction of blood vessels and blood-brain barrier (BBB)/ blood-spinal barrier (BSCB) is the main cause of immune response-mediated neuroinflammation, which involves the activation of immune organs, immune cells and immune factors, leading to metabolic dysfunction such as damage to local neurons and glial cells. At present, acupuncture in the treatment of SCI has a significant effect in clinical application, the mechanism of action is related to the regulation of neuroinflammation-related immune cells, including the regulation of neutrophils, microglia, lymphocytes, astrocytes and other cell activation phenotype and cytokine expression, so as to effectively reduce the inflammatory microenvironment after SCI and promote the repair of nerve function. This paper systematically summarizes the recent domestic and foreign studies on the mechanism of neuro-immunoinflammation after SCI, and elaborates the feasibility basis of acupuncture as an effective neuro-immunotherapy, providing theoretical support for the widespread application of acupuncture as a multi-effect target therapy in clinical practice.
