| 目的：探讨筋骨针针刺肩三针与手三针治疗肩周炎的效果及对疼痛程度、肩关节活动度的影响。方法：将2019年5月~2023年5月收治的120例肩周炎患者依照随机数字表随机分为对照组与三针组各60例，对照组实施常规针刺疗法，三针组实施肩三针+手三针针刺疗法。比较两组临床疗效，治疗前后中医症状积分、肩关节疼痛度和活动度情况及血清生化指标[白细胞介素-6（IL-6）、转化生长因子-β（TGF-β）和前列腺素E2（PGE2）]变化。结果：三针组治疗后总有效率96.67%（58/60）高于对照组86.67%（52/60），有统计学意义（P<0.05）；治疗后，两组中医症状积分、IL-6、TGF-β和PGE2水平均低于治疗前，肩关节活动角度高于治疗前，且三针组与对照组间比较，有统计学意义（P<0.05）；治疗第1 d、治疗第3 d、治疗第1周两组肩关节疼痛度评分逐渐降低（P<0.05），且三针组显著低于对照组（P<0.05）。结论：筋骨针针刺肩三针与手三针能治疗肩周炎效果显著，可有效减轻肩关节疼痛，提高活动度，改善临床症状与炎症反应，值得应用。 |

| Objective: To explore the effect of tendon-bone needle acupuncture with shoulder three needles and hand three needles in the treatment of scapulohumeral periarthritis and its influence on pain degree and shoulder range of motion. Methods: 120 patients with scapulohumeral periarthritis admitted from May 2019 to May 2023 were randomly divided into control group (60 cases) and three-needle group (60 cases) according to the random number table method. The control group carried out routine acupuncture therapy, while the three-needle group was treated with shoulder three-needle + hand three-needle acupuncture. The clinical efficacy, and TCM symptoms scores, shoulder pain degree and range of motion, and serum biochemical indicators [interleukin-6 (IL-6), transforming growth factor-β (TGF-β) and prostaglandin E2 (PGE2)] before and after treatment were compared between groups. Results: The total effective rate of treatment in three-needle group with 96.67% (58/60) was higher than 86.67% (52/60) in control group (P<0.05). After treatment, the scores of TCM symptoms and levels of IL-6, TGF-β and PGE2 in both groups were lower than those before treatment while the shoulder range of motion was higher than that before treatment, and the differences between three-needle group and control group were statistically significant (P<0.05). On the 1st day, 3rd day and 1st week of treatment, the shoulder pain score in the two groups was gradually decreased (P<0.05), and the score was significantly lower in three-needle group than that in control group (P<0.05). Conclusion: Tendon-bone needle acupuncture with shoulder three needles and hand three needle has a significant effect on scapulohumeral periarthritis, and it can effectively relieve shoulder pain, improve range of motion, improve clinical symptoms and inflammatory response. |
